Instructions On Q-GAP™ Scoring
Fill out the Questionnaire!
The Q-GAP™ questionnaire provides a list of symptoms. You simply identify whether you ever experience each and, if so, the frequency and intensity with which you experience them. Here are some guidelines:
Frequency of Symptoms
| |
You may experience certain symptoms only under specific conditions - for example, during a bout of influenza or certain times of the year (seasonal allergies). Please acknowledge that you do experience these symptoms, no matter what the cause or frequency.
|
Almost never | - |
isolated or occasional occurence |
Occasional | - |
experience periodically but does not significantly affect
day-to-day life |
Moderate | - |
bothersome but does not significantly affect ongoing quality
of life |
Almost Always | - | impacts day-to-day life and significantly detracts from quality of life |
Intensity of Symptoms
| |
Please identify the intensity of these symptoms, no matter what the cause or frequency. |
Mild | - |
symptom is noticeable but does not impede day to day functioning
in any significant way |
Moderate | - | symptom is present sufficiently to be irritating and requires relief |
Severe | - | symptom is significant enough to seek or have received medical attention |
How to read the Q-GAP™
- The Green "Qo" line represents OPTIMAL health throughout
life - disease-free and without symptoms of premature aging that
impair everyday functioning and enjoyment of life. Evidence shows
that with effective health risk identification and the right health
strategies (including nutrition, lifestyle and preventive medications
where necessary) individuals can expect to live longer, enjoyable
and functional lives.
- The Yellow zone represents SUB-OPTIMAL health where inherent
risks begin to emerge as symptoms. Unless actively addressed,
this could lead to premature disability and disease. It is important
to act with urgency before drifting into the Red zone.
- The Red "Qt" line represents TYPICAL health - typical
of so many who fail to acknowledge and act on the importance of
managing the key drivers of their health BEFORE premature aging
sets in. Once disease conditions take hold they are more difficult
to arrest and reverse. Conditions such as diabetes (often induced
by excess weight) then compound and lead to other debilitating
symptoms and conditions, such as heart disease, further lowering
quality of life.
- The Q-GAP™ is the difference between the Quality of
Life experienced by so many typical individuals and the Optimal
Quality of Life they could experience if they begin to manage
their health before serious deterioration sets in. It is based
on the principle that undesirable physical, emotional and spiritual
symptoms underlie the root of declaring one's deteriorating "quality
of life".
Score YOUR OWN Q-GAP™ by completing the questionnaire. Simply follow the instructions and we will give you the result immediately.
