  We ask for essential information for your medical file and to enable us to communicate efficiently with you. This will be stored confidentially in your password-protected Electronic Medical Record. Your personal and family health history and related information are critical to designing your Program and confirming the most appropriate testing.

  You will receive a user name and password to access this diet analyzer in a secure part of this website. Please log in and record at least 5 days of food and beverage intake. You can generate your own reports for this period if you wish.
   
• Enrollment
  For legal and privacy reasons we ask that this document be signed in hard copy format in our offices. We will have a printed version ready for you. We have included it here so that you have the time to review it at your convenience.

• Q-GAP™ Questionnaire
  The Q-GAP™ is a tool we have developed as a quick and very effective way to measure your "Quality of Life" index. It not only provides our Health Team with important information about your health and factors affecting it, but you will find it interesting. Take the test before your Enrollment Consultation here and your score will be available to you within seconds. Allow yourself about 10 minutes to complete the Questionnaire.

Information about Enrollment & Testing

Enrollment Consultation
Conducted by one of our senior physicians. The purpose is to discuss with you which Program best meets your needs. If you have already decided to enroll in a specific Program you have the option to schedule your Consultation for the same day as your testing. If you would prefer to use the consultation to help you reach a decision on our Programs, you will be charged a fee of $300, which will be waived should you proceed to enroll.

As part of this initial consultation we will review your Q-GAP™ Score, measure your BMI - Body Mass Index (body fat, height, and weight relationship) and test your blood pressure. At the conclusion of the consultation the physician will explain and confirm which tests you will require.

It is very important that you complete all questionnaires we provide prior to your Enrollment Consultation.
Please expect to be in our clinic for about 1.5 hours, with the actual consultation being 30-40 minutes.

Testing
We conduct all consultations and most of our testing at our Executive Center in the Yonge Corporate Centre, 4120 Yonge Street, Toronto. Please see the map and directions at www.scientahealth.com/aboutus/locations.

Diagnostic Imaging procedures (CT, MRI and other DI testing) are performed by special arrangement at select locations. We schedule all appointments for you.

Fitness Testing is performed by Scienta Health approved fitness instructors at an executive studio.
